1. your contact info;
2. identification of the copyrighted work;
3. the specific URL(s) of the allegedly infringing material;
4. a good-faith statement that use is not authorized;
5. a statement under penalty of perjury that your notice is accurate and you are the owner or authorized to act;
6. a physical or electronic signature.

We may remove or disable access to reported material and notify the user.

Counter-Notice

If your content was removed by mistake or misidentification, send a counter-notice including:

1. your contact info;
2. identification of the removed material and where it appeared;
3. a statement under penalty of perjury that the removal was a mistake;
4. consent to jurisdiction of the federal court for your district (or Massachusetts, if outside the US) and acceptance of service from the complainant;
5. your signature.

Upon valid counter-notice, we may restore the material unless the complainant files court action.

Repeat Infringers

Accounts with multiple valid infringement notices may be terminated.

Notice Distribution

We may forward notices (including personal information) to the user and to public databases that track DMCA notices.
